Diabetes patients can live long and healthy with excellent quality of life, if diabetes is diagnosed at an early stage and managed well.
As diabetes is a complex disease with the potential to affect all organs and organ systems, modern diabetes management must be holistic and integrated. Good diabetes treatment is focused on three corner stones:
- Modern medication for effective control of blood sugar, blood cholesterol and hypertension
- Regular organ screening to assure early diagnosis of organ complication (eyes, skin, heart…)
- Lifestyle changes, including regular physical exams and healthy diet
Unpleasant side effects and complications can be avoided in many cases or at least delayed, if all three elements of diabetes management are considered thoroughly. For the patients it is difficult however to coordinate all needed appointments without missing important follow up visits and control exams.
Diabetes Registry, Interactive Diabetes Patient Education
At Amber Clinics, diabetes patients can register in a Diabetes Management Program. Enrolled patients will be followed up by an electronic program, all needed and arranged follow up visits and screening exams are tracked, and the patients reminded in a timely manner of their appointment.
Amber clinics also offer regular patient education to diabetic patients. This education program consists of a series of interactive sessions, each attended by 8 to 12 concerned patients and moderated by experienced diabetes nurses and/or physicians.
We are using the “Diabetes Conversation Map Education Tools”, as recommended by the American Diabetes Association (with the friendly support of Lilly©): under moderation of the educator nurse, all relevant elements of successful diabetes management are discussed between the attending diabetes patients. Important knowledge will be transferred and personal experiences exchanged between the patients.
Scientific studies have been shown that diabetes patients attending this kind of education have better outcome, less complication and better quality of life.
As Amber Clinics consider Diabetes Registry and Interactive Diabetes Education as a Corporate Social Responsibility, attendance of both programs is free of cost for everybody.
